Network access request form

Use this form to:

• Request an IP address
• Activate a network port
• Obtain a blue network cable

If your network port (ie., any free and conveniently close network wall plate socket) does not have a bright orange sticker directly near it then you need to say "Yes" to "Activate port" and you must fill in details so that the appropriate port can be activated. To activate a network port we need at least the port label (eg., 104A-C1-PP8) and the single or double digit port number written next to it. If you need a blue network cable you may say so here. After you have submitted the form you will be contacted by Physics IT Support personnel via e-mail or telephone.
